Advanced Directives

The Spartan Health Surgicenter is required by law to request and provide information on living wills. If you have a living will and/or a durable power of attorney, please give copies of these documents to the Spartan Health Surgicenter receptionist to be included in your medical record. These documents would be forwarded with your records in the unlikely event that admission to a hospital is required. If you have any questions regarding our policies, please call the Spartan Health Surgicenter office.
